Job summary

One Senior Care is seeking a Medical Assistant – Clinical Documentation Support to help prepare provider-facing documentation for PACE participants. You will organize existing medical records, prepare packets, manage work queues, and ensure clinicians have the information needed for scheduled visits.

The role emphasizes accuracy, organization, and proficiency with electronic medical records and medical coding.

Qualifications

• High School Diploma or GED is required.
• Medical Assistant training certificate or equivalent healthcare support experience preferred.
• Previous medical coding experience with a CC focus required.
• Experience reviewing provider charts and understanding documentation practices required.
• Experience with electronic medical records required.
• Experience with frail/elderly populations (PACE geriatrics or value-based care) is a plus.

Responsibilities

• Gather and organize existing medical records and approved clinical documentation for provider review.
• Prepare provider-facing documentation packets and templates before scheduled Participant visits.
• Review queued items and retrieve approved reports and supporting documentation.
• Organize hospital, ED, SNF, labs, imaging, meds, vital signs, and care plan information.
• Ensure materials are complete, factual, organized, and available within timelines.
• Maintain HIPAA-compliant documentation and confidentiality.
• Collaborate with providers, Medical Records, CDI Nurses, and scheduling teams.
• Escalate unclear or audit-sensitive information to the clinical lead.
