Job summary

Tuesday Health, a value-based palliative care provider, seeks a Complex Care Navigator RN in Massachusetts. This role leads care coordination, administers assessments, develops care plans, and drives a coordinated, technologically enabled member experience.

The RN will work within a multidisciplinary team, learn the company’s electronic systems, build trusting relationships with members, and ensure timely escalation when needed. Remote position with occasional onsite sessions across MA.

Qualifications

• Active and unrestricted RN license in Massachusetts.
• Experience in a clinical/medical setting preferred.
• Strong communication and organizational skills.
• Ability to work in a multidisciplinary care model.

Responsibilities

• Administer assessments and screening tools within the care model.
• Interpret results and prioritize member needs.
• Lead care plan development within the care team.
• Participate in internal and joint rounds for coordinated care.
• Learn and use Tuesday Health electronic systems to support care.
