Job summary

Tuesday Health is seeking a Complex Care Navigator RN to lead person-centered care for individuals with serious illness. You will administer assessments, interpret results, and help develop care plans within a multidisciplinary team, using our electronic systems to coordinate care and ensure a seamless member experience.

The ideal candidate holds an active MA RN license, excels in communication and organization, and can adapt to evolving processes.

Qualifications

Active and unrestricted RN license in Massachusetts.
Experience in clinical/medical settings preferred.
Experience in multidisciplinary care models.
Strong communication and organizational skills.
Ability to multitask and work under pressure.
Comfort with change and ambiguity.
Valid driver’s license with safe driving record.

Responsibilities

Administer and interpret assessments and screening tools within the clinical model.
Interpret responses and prioritize needs based on assessments.
Be accountable for care plan development within the multidisciplinary team.
Lead internal and joint rounds to ensure optimal member experience.
Learn Tuesday Health’s electronic systems and tools to deliver coordinated care.
Build strong, trusting relationships with every member with empathy as the foundation.
Serve as quarterback of the multidisciplinary team transforming care for members with serious illness.
