Job summary

Tuesday Health, LLC is seeking a Complex Care Navigator RN who will deliver comprehensive, person-centered care to individuals with serious illness. You will administer assessments, interpret responses, and lead care coordination within a multidisciplinary team to develop individualized care plans and drive optimal member experiences.

Qualifications

• Active and unrestricted RN license in Massachusetts with no board actions.
• Experience in clinical/medical settings preferred.
• Experience in a multidisciplinary care model with diverse providers.
• Strong communication and organizational skills.
• Ability to multitask and work under pressure while maintaining patient experience.
• Know when to elevate care in emergent situations.
• Proficiency in navigating documentation systems and learning new tools.
• Valid driver’s license with a safe driving record.
• State minimum automobile insurance coverage.
• Comfort with change and ambiguity and rapid business evolution.

Responsibilities

• Administer multiple assessments and screening tools within the clinical model over the care timeline.
• Interpret responses and prioritize needs based on assessments.
• Lead care plan development within a multidisciplinary team.
• Lead internal and joint rounds to optimize the member experience.
• Learn and use Tuesday Health electronic systems and tools to deliver coordinated care.
• Build strong, trusting relationships with members through listening and empathy.
• Serve as the quarterback of the multidisciplinary team to transform care for seriously ill members.
