Job summary

Pacific Health Group in California operates the Enhanced Care Management (ECM) program and is seeking a Licensed Vocational Nurse (LVN) to support high-risk Medi-Cal members. The role blends remote work with field visits, under RN supervision, to coordinate care, educate members, and reconcile medications.

The LVN will collaborate with nurses, care managers, behavioral health staff, and community partners, ensuring timely documentation and adherence to Medi-Cal/ECM guidelines.

Qualifications

• Current and unrestricted California LVN license.
• Minimum one year of clinical healthcare experience.
• Experience working with underserved or vulnerable populations preferred.
• Strong understanding of care coordination and chronic disease management principles.
• Excellent communication, organizational, and interpersonal skills.
• Must reside in or within a reasonable commuting distance of Hiring County.

Responsibilities

• Support CalAIM ECM care coordination and member outreach.
• Assist with member assessments, health education, and medication reconciliation.
• Coordinate field visits, home visits, and provider-site visits.
• Collaborate with RNs, Lead Care Managers, Community Health Workers, and providers.
• Maintain timely documentation in the electronic health record and care management systems.
• Protect member confidentiality and ensure HIPAA compliance.
