Job summary

UnitedHealth Group is seeking a LTSS Case Manager to perform intake, assessments, and ongoing care coordination for members in the assigned territory. The role involves home visits and coordination with supervisors to ensure quality outcomes.

The position requires travel up to 75% within the territory, with some remote work options for residents in CO counties. The ideal candidate will have a human services background and strong MS Office skills.

Qualifications

• Bachelor's degree in human behavioral science fields such as human services, social work, psychology, or public health.
• 5+ years of relevant LTSS experience or a combination of education and experience.
• Proficiency with MS Office (Word, Excel, Outlook).

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans and ongoing case management.
• Document contacts and case activities in the prescribed system and monitor health/welfare.
• Conduct quarterly and semiannual face-to-face visits and update assessments/care plans as needed.
