Job summary

UnitedHealthcare is seeking a Long Term Services and Supports (LTSS) Case Manager to support case management goals and participate in case conferences with the Supervisor. You will help coordinate care, document contacts, and monitor member health and welfare in a compliant system.

The role requires about 50% travel to members’ homes or long-term care facilities. You will work in Delta, CO with a hybrid schedule, visiting the Delta office two days per week, and maintain strict confidentiality

Qualifications

• Bachelor's degree in human behavioral science fields such as human services, social work, psychology, or public health.
• 5+ years of relevant LTSS experience.
• 1+ years of MS Office experience.
• Access to reliable transportation and ability to travel up to 50% within assigned territory.
• Reside locally to Delta, Hinsdale, or Gunnison County, CO or surrounding areas.
• Driver's License.

Responsibilities

• Intake/screening/referral, assessment/reassessment, development of support plans and ongoing case management.
• Maintain confidentiality of patient information and adhere to the handbook requirements.
• Conduct quarterly monitoring contacts with members or as required by LTSS rules.
• Perform face-to-face contacts at least every six months and update assessments and care plans.
• Complete annual or more frequent reassessments as needed.
• Monitor service delivery within the Support Plan and PAR.
• Attend Department-provided CMA training and in-service sessions.
• Ensure work aligns with CMA Contract requirements; provide accurate, timely, complete work.
• Refer suspected abuse or mistreatment to protective services immediately.
• Take on special assignments and assist with system assessments as needed.
• Travel between locations; if driving, comply with Colorado licensure and insurance.
