Job summary

Tuesday Health in Peabody, MA is seeking a Licensed Practical Nurse to join our complex care team. The LPN will work with nurse practitioners and RNs to perform clinical assessments, develop and execute care plans, and support coordinated rounds to optimize the member experience.

The ideal candidate holds an active MA LPN license, has experience in clinical settings, and demonstrates strong communication, organization, and adaptability in a dynamic multidisciplinary environment.

Qualifications

• Licensed Practical Nurse license in Massachusetts without board action.
• Experience in clinical/medical settings preferred.
• Experience in multidisciplinary care models and care management support.
• Strong communication and organizational skills; able to multitask and maintain a positive member experience.

Responsibilities

• Administer clinical assessments and screenings within the care model.
• Prioritize member needs based on assessments and collaborate with the care team.
• Develop and execute individualized care plans within the multidisciplinary team.
• Participate in care rounds to ensure coordinated and optimal member experience.
• Learn and utilize Tuesday Health electronic systems and tools for coordinated care.
• Build trusting relationships with members through listening and empathy.
