Tuesday Health is hiring a Licensed Practical Nurse in Peabody, Massachusetts, to support serious-illness care. The LPN will perform clinical assessments, participate in multidisciplinary rounds, and help develop and execute individualized care plans with nurse practitioners and RNs.

The role requires an active MA LPN license, strong communication, and the ability to navigate electronic systems. This hybrid position offers remote work with occasional onsite sessions and a comprehensive benefits

Qualifications

• Active and unrestricted LPN license in Massachusetts.
• Experience in clinical/medical settings.
• Experience in a multi-disciplinary care model.
• Excellent communication and organizational skills.
• Ability to multitask and work under pressure.
• Strong technical skills to navigate documentation systems.
• Valid driver's license and ability to travel as needed.
• Comfort with change and ambiguity.

Responsabilités

• Administer assessments and screenings as part of the care plan.
• Collaborate with the interdisciplinary team to develop care plans.
• Participate in care rounds and ensure coordinated member experience.
• Leverage electronic systems to document and coordinate care.
• Build trusted relationships with members through active listening.
• Prioritize member needs based on assessments and results.
• Act as quarterback of the care team for complex cases.
