Tuesday Health, LLC is seeking a Licensed Practical Nurse (LPN) in Peabody, Massachusetts to deliver coordinated, compassionate care for seriously ill members. The LPN will perform clinical assessments, support care plans, and collaborate with nurse practitioners and RNs.

Qualifications

• Active and unrestricted LPN license in the State of Massachusetts.
• Experience in clinical/medical settings preferred.
• Experience in multi-disciplinary care models is a plus.
• Strong communication and organization skills.
• Ability to drive during daytime and nighttime, with a valid license.

Responsibilities

• Administer assessments and screenings within the clinical model.
• Prioritize needs based on assessment results and coordinate with the care team.
• Contribute to care plan development within the multidisciplinary team.
• Participate in rounds to ensure a coordinated member experience.
• Learn and use Tuesday Health electronic systems to support member care.
