Job summary

Tuesday Health in Winthrop, MA, seeks an LPN for Complex Care Navigator to coordinate care for seriously ill members. You will work with nurse practitioners and RNs to assess, plan, and execute individualized care within a multidisciplinary team.

Expect to lead care plans, participate in rounds, and learn Tuesday Health’s electronic tools to deliver a seamless, person-centered experience. A MA LPN license and strong communication are required.

Qualifications

• Active and unrestricted LPN license in Massachusetts with no board action.
• Clinical/medical setting experience preferred.
• Experience in multidisciplinary care models with providers and care management.
• Strong communication and organizational skills.
• Ability to multitask under pressure while maintaining patient experience.
• Able to escalate appropriately in emergent situations and stay calm.
• Strong technical skills to navigate documentation systems and learn new tools.
• Valid driver’s license and insurance coverage.

Responsibilities

• Administer assessments and screening tools within the clinical model with team support.
• Prioritize needs based on assessments and coordinate with the care team.
• Own care plan development within the multidisciplinary team.
• Participate in rounds to ensure a coordinated member experience.
• Learn Tuesday Health’s electronic systems to deliver consistent care.
• Build trusting relationships with members through listening and empathy.
• Act as the quarterback of the care team transforming care for members.
