Job summary
Tuesday Health in Lynn, Massachusetts is seeking a Licensed Practical Nurse to join our Complex Care Navigator team. Qualifications
- Active and unrestricted LPN license in Massachusetts with no board actions.
- Clinical/medical setting experience preferred.
- Experience in a multi-disciplinary care model is a plus.
- Strong communication and organizational skills.
Responsibilities
- Administer assessments and screenings with support from nurse practitioners and registered nurses.
- Prioritize needs based on assessments and coordinate with the interdisciplinary care team.
- Contribute to care plan development within the multidisciplinary team.
- Participate in rounds to ensure coordinated and optimal member experience.
- Learn and use Tuesday Health electronic systems to deliver coordinated care.
- Build trusting relationships through listening and empathy.
- Act as quarterback of the multidisciplinary team to transform care.
