Job summary

Tuesday Health in Chelsea, Massachusetts is seeking a Licensed Practical Nurse to join our Complex Care Navigator team. You will collaborate with nurse practitioners and RNs to perform assessments, develop care plans, and ensure coordinated, person-centered care for members with serious illnesses.

This role emphasizes empathy, effective communication, and seamless use of our electronic systems to optimize the member experience. A valid MA LPN license and drive to various sites are required.

Qualifications

• Active and unrestricted LPN license in Massachusetts
• Experience in clinical/medical setting
• Experience in a multi-disciplinary care model
• Strong communication and organization skills
• Ability to multitask and work under pressure
• Valid driver's license and safe driving record; auto insurance
• Comfort with change and ambiguity; expect rapid business evolution

Responsibilities

• Administer multiple assessments and screening tools with NP/RN support
• Prioritize needs based on assessment results and coordinate care
• Develop care plans within the multidisciplinary team
• Participate in rounds to ensure coordinated member experience
• Learn and use Tuesday Health electronic systems and tools
• Build trusting relationships with members through empathy
• Act as quarterback of the multidisciplinary team to transform care
