Tuesday Health in Lawrence, Massachusetts is seeking a Licensed Practical Nurse to join our value-based palliative care team. The LPN will perform clinical assessments, support care plans, and collaborate with nurse practitioners and RNs to coordinate compassionate care for members with serious illnesses.

This role emphasizes trust, empathy, and effective communication. A Massachusetts LPN license is required, along with comfort using electronic systems and contributing to a multidisciplinary

Qualifications

• Active and unrestricted LPN license in Massachusetts.
• Experience in clinical/medical setting preferred.
• Experience in a multi-disciplinary care model.
• Strong communication and organization skills.
• Ability to multitask and work under pressure while maintaining member experience.
• Ability to drive during daytime, nighttime, or inclement weather; valid driver’s license and insurance.
• Comfort with change and ambiguity in a rapidly evolving organization.

Responsibilities

• Administer multiple assessments and screenings within the clinical model with NP/RN support.
• Prioritize needs based on assessment results and coordinate with the interdisciplinary care team.
• Be accountable for care plan development within the multidisciplinary team.
• Participate in rounds to ensure a coordinated member experience.
• Learn and use Tuesday Health electronic systems and tools.
• Build trusting relationships with members through empathy and active listening.
• Serve as the quarterback of the multidisciplinary team transforming care for members with serious illness.
