Job summary

Tuesday Health is a value-based palliative care provider transforming serious illness and end-of-life care. We seek a Licensed Practical Nurse to join the Complex Care Navigator team in delivering coordinated, compassionate care for members with serious illnesses.

The LPN will work with nurse practitioners and RNs, perform clinical assessments, and contribute to individualized care plans. Remote work is offered with periodic onsite sessions.

Qualifications

• Active and unrestricted LPN license in Massachusetts.
• Experience in clinical/medical settings preferred.
• Experience in a multi-disciplinary care model is a plus.
• Strong communication and organizational skills.
• Ability to multitask and maintain positive member experience.
• Ability to adapt to learning new systems as the company grows.

Responsibilities

• Administer multiple assessments and screening tools within the clinical model with support from clinicians.
• Prioritize needs based on assessments and coordinate with the interdisciplinary care team.
• Be accountable for care plan development within the team.
• Participate in rounds to ensure coordinated member experience.
• Learn and navigate Tuesday Health electronic systems and tools to deliver coordinated care.
• Build trusting relationships with members through listening and empathy.
