Job summary

Tuesday Health, LLC is seeking a Licensed Practical Nurse (LPN) in Lawrence, Massachusetts to join our compassionate palliative care team. You will assist with clinical assessments, coordinate with the care team, and support individualized care plans to improve member experiences.

Strong communication, empathy, and multitasking are essential. MA LPN licensure and reliable transportation are required; remote work is available with periodic onsite sessions.

Qualifications

• Active and unrestricted MA LPN license without board action.
• Clinical/medical setting experience preferred.
• Strong communication and organizational skills; multitasking under pressure.

Responsibilities

• Administer assessments and screenings with support from RNs and NPs.
• Develop and implement care plans within the multidisciplinary team.
• Participate in rounds and coordinate care using Tuesday Health systems.
• Build trusting relationships with members through listening and empathy.
• Serve as quarterback of the care team to improve member experience.
