Qualifications

• Active and unrestricted Licensed Practical Nurse license in the State of Massachusetts.
• Experience in clinical/medical settings preferred.
• Experience in a multi-disciplinary care model working across providers and care management.
• Strong communication and organizational skills.
• Ability to multitask and maintain a positive member experience.
• Ability to respond professionally in emergent situations.
• Tech-savvy with different documentation systems and a willingness to learn new tools.

Responsibilities

• Administer multiple assessments and screening tools within the clinical model with support from NPs and RNs.
• Prioritize needs based on assessment results and coordinate with the interdisciplinary team.
• Be accountable for care plan development within the care team.
• Participate in rounds to ensure a coordinated member experience.
• Learn Tuesday Health electronic systems and tools to deliver a coordinated member experience.
• Build trusting relationships with members through listening and empathy.
