Job summary

Tuesday Health is seeking a Licensed Practical Nurse in Lawrence, MA to deliver coordinated, compassionate care for members with serious illness. You will work with nurse practitioners and RNs to assess, screen, and craft individualized care plans within a multidisciplinary team.

Responsibilities include rounds participation, learning Tuesday Health electronic systems, and building trusting relationships with members to ensure a seamless, person-centered experience.

Qualifications

• Active and unrestricted LPN license in Massachusetts.
• Clinical/medical setting experience preferred.
• Experience in a multidisciplinary care model.
• Strong communication and organizational skills.
• Ability to multitask and work under pressure.
• Valid driver's license with safe driving record.
• State minimum automobile insurance coverage.

Responsibilities

• Administer multiple assessments and screening tools within the clinical model with support from clinicians.
• Prioritize needs based on assessment results and coordinate with the interdisciplinary care team.
• Be accountable for care plan development within the multidisciplinary team.
• Participate in internal and joint rounds to ensure a coordinated member experience.
• Learn the Tuesday Health electronic systems and tools to deliver a coordinated experience.
• Build a strong, trusting relationship with every member through listening and empathy.
• Serve as the quarterback of the multidisciplinary team transforming care for members with serious illness.
