Tuesday Health is seeking a Licensed Practical Nurse (LPN) in Lawrence, MA to join a value-based palliative care provider group. The LPN will support assessments, care coordination, and care plan execution for members with serious illnesses, collaborating with nurse practitioners and RNs to deliver compassionate, patient-centered services.

Qualifications

• Active and unrestricted LPN license in Massachusetts without board action.
• Experience in clinical/medical settings preferred.
• Ability to work in a multidisciplinary care model.

Responsabilités

• Administer assessments and screenings per care plan with support from the care team.
• Prioritize member needs based on assessments and collaborate on care plans.
• Participate in rounds and coordinate care using electronic systems.
• Build trusting relationships and engage members with empathy.
• Act as a quarterback of the team to transform care for seriously ill members.
