Tuesday Health is hiring a Complex Care Navigator LPN to deliver coordinated, compassionate care for members with serious illnesses. You will work with nurse practitioners and RNs, perform assessments, and help develop individualized care plans that emphasize empathy and trust.

The role requires an active MA LPN license, strong communication skills, and comfort with electronic systems. Remote work is offered with onsite sessions to foster collaboration and team alignment.

Qualifications

• Active, unrestricted LPN license in Massachusetts.
• Clinical/medical setting experience preferred.
• Experience in multidisciplinary care models and teams.
• Strong communication and organizational skills.
• Ability to learn new systems quickly.

Responsibilities

• Administer assessments and screenings with NP/RN support.
• Prioritize needs based on assessment results and team input.
• Develop and execute individualized care plans within the care team.
• Participate in care rounds to ensure coordinated member experience.
• Learn and use Tuesday Health electronic systems and tools.
• Build trusting relationships with members through empathy.
• Serve as quarterback of the multidisciplinary team overseeing care.
