Job summary

Tuesday Health is seeking a Licensed Practical Nurse to join our Dayton, Ohio team. The LPN will perform clinical assessments and screenings, collaborating with nurse practitioners and registered nurses to develop and execute individualized care plans.

You will participate in rounds, utilize electronic tools, and maintain strong, trusting relationships with members to deliver person-centered care across home-based settings. Flexibility for daytime or nighttime shifts is required.

Qualifications

• Active and unrestricted LPN license in Ohio.
• Experience in clinical/medical settings preferred.
• Experience in a multi-disciplinary care model is a plus.
• Strong communication and organizational skills.

Responsibilities

• Administer clinical assessments and screenings with support of NPs and RNs.
• Prioritize member needs based on assessments and contribute to care plans.
• Participate in rounds to ensure coordinated, quality member experience.
• Learn and navigate Tuesday Health's electronic systems and tools.
• Build trusting relationships with members through empathy and active listening.
• Serve as quarterback of the multidisciplinary team to transform care.
