Job summary

Tuesday Health seeks a Licensed Practical Nurse for Warren/Youngstown, Ohio to join our complex care navigator team delivering value-based palliative care. The LPN will collaborate with nurse practitioners and RNs to perform assessments, support care planning, and coordinate multidisciplinary care for members with serious illness.

You will use electronic systems to document care, build trust with members, and participate in rounds to optimize the member experience.

Qualifications

• Active and unrestricted LPN license in Ohio with no board actions.
• Clinical/medical setting experience preferred.
• Experience in multi-disciplinary care models and across providers.
• Strong communication and organization skills; multitask under pressure.
• Ability to learn new documentation systems; valid driver's license and insurance.

Responsibilities

• Administer assessments and screenings within the care model with support.
• Prioritize needs based on assessments and coordinate with the care team.
• Develop and implement care plans within the multidisciplinary group.
• Participate in rounds to ensure coordinated member experiences.
• Learn and use Tuesday Health electronic systems to deliver care.
