Tuesday Health is seeking a Licensed Practical Nurse (LPN) in Massachusetts to contribute to complex care navigation for seriously ill members. The LPN will perform clinical assessments, collaborate with nurse practitioners and RNs, and develop care plans within a multidisciplinary team.

Qualifications

• Active and unrestricted LPN license in Massachusetts without board action.
• Clinical/medical setting experience preferred.
• Experience in a multi-disciplinary care model with network providers and care management support.
• Strong communication and organizational skills.
• Ability to multitask and work under pressure while maintaining a positive member experience.
• Ability to escalate appropriately in emergent situations.
• Strong technical skills to navigate different documentation systems and learn new ones.
• Strong written communication skills.
• Ability to drive during daytime, nighttime, or inclement weather.
• Valid driver’s license with a safe driving record; state minimum auto insurance.
• Comfort with change and ambiguity in a rapidly evolving business.

Responsabilités

• Administer multiple assessments and screening tools with NP/RN support.
• Prioritize member needs based on assessments and coordinate with the care team.
• Be accountable for care plan development within the multidisciplinary team.
• Participate in rounds to ensure coordinated member experience.
• Learn and use Tuesday Health electronic systems and tools.
• Build trusting relationships with members through listening and empathy.
• Act as the quarterback of the multidisciplinary team to transform care.
