Job summary

Tuesday Health seeks a Licensed Practical Nurse in Lawrence, MA to join its value-based, interdisciplinary palliative care team. The LPN will perform clinical assessments, collaborate on care plans with NPs/RNs, and participate in rounds.

The role emphasizes compassionate, person-centered care and uses electronic systems for coordination. Remote work is offered with onsite sessions; salary up to $95,000 per year.

Qualifications

• Active and unrestricted LPN license in MA.
• Clinical/medical experience preferred.
• Experience in multidisciplinary care model.
• Strong communication and organization skills.
• Ability to multitask under pressure.
• Willingness to drive and maintain a valid license.
• Comfort with change and ambiguity.

Responsibilities

• Administer assessments and screenings with NP/RN support.
• Prioritize needs and support interdisciplinary care team.
• Develop and execute care plans within the multidisciplinary team.
• Participate in rounds to coordinate the member experience.
• Learn and use Tuesday Health electronic systems.
• Build trusting relationships with members.
• Serve as quarterback of the multidisciplinary team transforming care.
