Job summary

Cadence is hiring a Clinical Navigator (Licensed Practical Nurse) to provide virtual chronic care support for Medicare patients. This full-time remote position requires an active compact multi-state LPN/LVN license and at least 5 years of clinical experience. Responsibilities include conducting patient outreach, coaching, education on disease management, and documenting interactions in the Cadence platform.

The anticipated compensation is $26 – $31 per hour, along with comprehensive benefits such as health coverage and a 401(k) plan.

Qualifications

• An active compact multi-state LPN/LVN license is required.
• At least 5 years of clinical experience as a Licensed Practical Nurse is required.
• Remote work readiness (including stable high-speed Wi-Fi) is required.
• Fluency in Spanish is preferred.
• Experience providing patient education and support to individuals managing chronic conditions is preferred.

Responsibilities

• Conduct outbound telephonic patient outreach and provide one-on-one coaching to patients managing chronic conditions.
• Help patients execute personalized care plans and achieve their goals.
• Conduct comprehensive assessments of patients' health status.
• Educate patients on disease management and medication adherence.
• Monitor patients' progress through regular check-ins and remote monitoring tools.
• Document patient interactions and identify barriers to care.
