CareHarmony is seeking an experienced Licensed Practical Nurse to join our fully remote Care Coordination team. This 100% remote role requires a minimum of 3 years of direct patient-facing experience and thrives in a fast-paced environment, with a strong focus on closing gaps in chronic care management.

You will manage patient education, medication coordination, and referrals, coordinating resources and ensuring timely delivery of Home Health and related services.

Qualifications

• Active LPN license in multi-state/compact.
• Proficiency with Microsoft Office Suite.
• Excellent written and verbal communication skills.

Responsabilités

• Manage patient census with a resolution-driven approach to close gaps in care.
• Identify and coordinate community resources for patients.
• Provide patient education and health literacy on chronic condition management.
• Perform medication management including reconciliation and adherence.
• Assist in timely delivery of services including Home Health, DME, and Home Infusion.
• Manage referrals and appointment scheduling.
