Qualifications

• At least 3 years of direct patient-facing work experience.
• Self-motivated with impeccable attention to detail.
• Experience identifying resources for chronic care management.

Responsibilities

• Manage patient census and address clinical gaps.
• Identify community resources to benefit patients.
• Provide patient education on chronic conditions.
• Perform medication management and reconciliation.
• Assist in timely delivery of Home Health and other services.
• Create an open dialogue with patients to understand needs.
• Manage referrals and appointments.
