Job summary

CVS Health is hiring a Bilingual Behavioral Health Case Manager (Licensed) to support Medicare members from home. This full-time, remote role emphasizes telephonic care coordination, assessments, and care planning across providers and community resources.

You will use Motivational Interviewing, collaborate with internal teams, and document work in clinical systems. The position requires a licensed clinician, MSW or counseling degree, and Spanish-English fluency.

Qualifications

• Active, unrestricted behavioral health license in your state (LCSW, LPC, LMFT, LPCC, LISW, LSW, or equivalent).
• Master's Degree in Social Work or Counseling.
• 3+ years of direct clinical behavioral health experience.
• Bilingual in Spanish and English; read, write, and speak fluently.
• Strong assessment, care planning and member engagement skills.

Responsibilities

• Provide telephonic case management for Medicare members with behavioral health needs.
• Complete comprehensive assessments and develop individualized care plans.
• Utilize Motivational Interviewing techniques to engage members and support outcomes.
• Coordinate care with providers, health plan partners, and community resources.
• Assist members in accessing behavioral health, social and community support services.
• Maintain accurate documentation within clinical systems.
