Job summary

CVS Health is seeking a licensed Behavioral Health clinician for a 100% remote Clinical Case Manager role. You’ll navigate complex mental health and substance use needs, perform crisis intervention, and coordinate care with providers and community resources to improve member outcomes.

The role requires a master’s in a behavioral health field and active state licensure, with 3+ years of direct clinical experience. This is a full-time remote position offering Monday–Friday hours and no holidays.

Qualifications

• Master’s degree required and licensed behavioral health professional in state of residence.
• Active, unrestricted independent license (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC).
• 3+ years post-master’s clinical experience in hospital, ambulatory, or outpatient settings.
• Experience treating mental health and substance use disorders.
• Experience in case management and discharge planning.
• Ability to multitask while documenting in clinical systems.
• Knowledge of managed care, utilization review, and cost-effective care.

Responsibilities

• Conduct comprehensive behavioral health assessments using clinical tools, data analysis, and benefit plan reviews.
• Develop individualized care plans addressing behavioral and physical health needs.
• Coordinate care with providers, community organizations, and support services.
• Apply advanced clinical judgment to identify and mitigate risk factors impacting outcomes.
• Perform crisis intervention and ensure referrals and follow-up.
• Coordinate psychosocial wraparound services and connect members with community resources.
• Support cost-effective care through proactive care planning and resource utilization.
