Job summary

CVS Health is seeking a Clinical Case Manager - Behavioral Health (Licensed) for a 100% remote, United States-based role. The position is full-time with a Monday–Friday schedule, no weekends or holidays.

You will conduct behavioral health assessments, develop care plans, coordinate with providers, and employ motivational interviewing to improve member outcomes. Ideal candidates hold a master’s in Behavioral Health or related field, an active license, and at least three years of post-master’s

Qualifications

• Master’s degree in Behavioral Health or related field.
• Active unrestricted license (e.g., LCSW, LISW, LMFT, LPC, LCPC, LPCC, LMHC).
• 3+ years post-master’s clinical experience in hospital, ambulatory, or similar settings.
• 3+ years of experience with mental health and substance use disorders.

Responsibilities

• Conduct comprehensive behavioral health assessments using clinical tools and reviews.
• Develop individualized care plans addressing behavioral and physical health.
• Coordinate care with providers, community organizations, and support services.
• Apply advanced clinical judgment to identify and mitigate risk factors.
• Perform crisis intervention and ensure referrals and follow-up.
• Utilize Motivational Interviewing to engage members and promote change.
• Coordinate psychosocial wraparound services with community resources.
• Support cost-effective care through proactive planning and resource use.
