Job summary

CVS Health is hiring a Clinical Case Manager – Behavioral Health (Licensed) to assess members, develop care plans, and coordinate services across the continuum of care. You will apply clinical judgment for risk factors and barriers to care, and use Motivational Interviewing to promote long-term behavior change.

This fully remote role involves crisis intervention, collaboration with providers and care teams, and thorough documentation of care interactions.

Qualifications

• Master's degree in social work, counseling or related behavioral health field.
• Active, unrestricted behavioral health license (LCSW/LISW/LMSW/LCPC/LPC/LMFT/LMHC or equivalent).
• 3+ years post-master's direct clinical experience in behavioral health settings.
• 3+ years treating mental health and substance use disorders.
• 3+ years of crisis intervention experience and use of Motivational Interviewing.
• Strong ability to document while engaging members telephonically.

Responsibilities

• Conduct comprehensive assessments of members' behavioral and physical health needs.
• Develop individualized care plans to improve health outcomes and wellbeing.
• Coordinate care with providers, care teams and community resources.
• Apply clinical judgment to address risk factors and barriers to care.
• Engage members using Motivational Interviewing to support behavior change.
• Provide crisis intervention and ensure access to treatment and support services.
• Coordinate behavioral health and substance use services across the continuum of care.
• Document care and manage member interactions using clinical systems.
