Qualifications

• Active, unrestricted clinical license in your state.
• 3+ years of direct clinical experience post-master’s.
• 3+ years of knowledge of mental health and substance use disorders.
• 3+ years of crisis intervention and motivational interviewing.
• 3+ years of case management experience.

Responsibilities

• Conduct comprehensive clinical assessments integrating medical, behavioral health, and psychosocial needs.
• Develop, implement, and monitor individualized care plans with members and providers.
• Coordinate care with medical and behavioral health professionals for high-quality outcomes.
• Provide crisis intervention, timely referrals, and follow-up support when indicated.
• Engage members and promote healthy behaviors using motivational interviewing and coaching.
• Identify quality of care concerns and escalate appropriately.
• Maintain accurate documentation in alignment with standards.
