CVS Health is seeking a qualified Care Manager to work fully remote in the USA. This full-time role runs Monday–Friday, 8:00–5:00 EST with no weekends or holidays, using advanced clinical judgment to assess member needs and coordinate care across medical and behavioral health services.

Qualifications

• 3+ years of direct clinical practice post-master’s degree in hospital or ambulatory settings.
• 3+ years knowledge of mental health and substance use disorders.
• 3+ years experience talking on the phone and typing at the same time.
• 3+ years experience using Motivational Interviewing with strong tech skills.
• Valid unrestricted independent professional behavioral health license per state regulations.

Responsibilities

• Assess members and evaluate needs/eligibility; determine approach to case resolution.
• Apply clinical guidelines and develop care plans.
• Coordinate psychosocial wraparound services; liaise with providers for optimal outcomes.
• Provide crisis intervention and follow-up to ensure appropriate treatment.
