Job summary

Apploi seeks a Lead Care Manager to partner with chronically ill members and their families to coordinate care across providers and community resources. This role focuses on engaging eligible members, implementing care plans, and reducing avoidable ER visits.

Qualifications

• Requires ability to collaborate with multidisciplinary teams.
• Must be able to visit patients and hospitals as needed.

Responsibilities

• Coordinate with member, family, and care team to ensure seamless services and avoid duplication.
• Engage eligible members and oversee ECM service delivery and care plan implementation.
• Provide services where the member lives or seeks care, within plan guidelines.
• Connect members to community resources and transportation as needed.
• Advocate for members with healthcare professionals and payers.
• Use motivational interviewing and trauma-informed care approaches.
• Coordinate discharge plans with hospital staff and caregivers.
• Attend meetings and participate in ongoing ECM program improvements.
