Job summary

Blehealth, Llc is seeking a Lead Care Manager to collaborate with chronically ill members, families, and providers. The role focuses on coordinating care, engaging eligible members, and implementing care plans across settings.

The position involves advocating for patients, leveraging motivational interviewing and trauma-informed care, and connecting members to community resources. It requires travel for home visits and hospital rounds, with mileage reimbursement.

Qualifications

• Associate's or bachelor’s degree in health science or related field preferred.
• Social Worker, LVN, or experience in case management is a PLUS.
• Must have and maintain your own personal vehicle for this role; valid driver’s license and proof of insurance required.

Responsibilities

• Coordinate with members, family/caregiver(s), providers, and community resources to ensure seamless care.
• Engage eligible members and oversee ECM services and care plan implementation.
• Promote timely access to appropriate care and reduce emergency room utilization and hospital readmissions.
• Educate members and family/caregiver(s) about relevant community resources.
• Monitor adherence to care plans and coordinate with hospital staff on discharge plans.
• Attend Lead Care Manager training courses and provide feedback for program improvement.
• Arrange transportation and accompany member to office visits as needed.
• Advocate for members with health care professionals and support self-management and decision-making.
