Job summary

Blehealth seeks a Lead Care Manager to coordinate care for chronically ill, high‑risk members within South Los Angeles. You will partner with members, families, providers, hospitals, and community resources to deliver high‑quality Enhanced Care Management services.

Responsibilities include coordinating care, overseeing care plans, arranging transportation, accompanying members to visits, monitoring treatment adherence, and promoting preventive care while adhering to HIPAA and Medi‑Cal MCP

Qualifications

• Valid California driver’s license and auto insurance.
• Reliable personal vehicle and ability to drive within 20-mile radius.
• Visit hospitals, member homes, and community locations as needed.
• Negative TB test and current CPR certification prior to hire.
• Live Scan fingerprint/background check.
• Ability to consistently meet daily productivity expectations.

Responsibilities

• Coordinate care across clinics, hospitals, specialists, and community agencies to ensure seamless service delivery.
• Oversee ECM services and ensure implementation of individualized care plans.
• Provide services where the member lives or seeks care; demonstrate cultural awareness and strong interpersonal skills.
• Assess medical, behavioral, and social needs; develop comprehensive care plans.
