Job summary

Molina Healthcare is seeking an IRIS Consultant to support Wisconsin IRIS participants remotely, with periodic home visits. You will help participants navigate the IRIS program, set personalized goals, and coordinate community resources to improve long-term care outcomes.

Qualifications

• Minimum 2 years healthcare experience with care coordination.
• Bachelor's degree in social work or related field (or four years of commensurate experience).
• Valid driver's license and reliable transportation.
• Ability to work independently with minimal supervision.
• Strong communication and interpersonal skills.
• Familiarity with IRIS program and long-term care concepts.
• Proficiency with HIPAA and confidentiality requirements.

Responsibilities

• Meet IRIS participants in person at least four times per year, with one annual home visit.
• Provide program orientation and explain participant rights and responsibilities.
• Help participants set goals and connect to community resources.
• Document activities in the IRIS data system (WISITs) within 48 hours of visits.
• Collaborate with IRIS Consultant Agency staff and attend monthly/weekly meetings.
• Assist with budget understanding and provider billing when needed.
