Job summary

Molina Healthcare seeks an IRIS Consultant to partner with Wisconsin IRIS participants, offering home-based support and guidance within the Medicaid long-term care program. The role is remote, with regular visits to participants' homes and communities.

This position requires a social services background, strong care coordination skills, and the ability to develop customized plans across employment, housing, health, safety and transportation.

Qualifications

• 2+ years in health care, care coordination preferred.
• Bachelor’s degree in a social services field or related area.
• Valid driver's license and reliable transportation.
• Ability to work independently and connect with diverse populations.

Responsibilities

• Meet IRIS participants in person at least four times per year, with one annual home visit.
• Educate participants on rights, responsibilities, and budgeting within IRIS.
• Develop customized IRIS plans addressing goals like employment, housing, health, safety and transportation.
• Document all activities and progress in the IRIS data system within 48 business hours.
• Collaborate with IRIS staff to implement participant plans and maintain confidentiality.
