Job summary

Molina Healthcare is seeking an IRIS Consultant to support Wisconsin participants in IRIS, remotely with required in-person visits. You will help participants develop personalized IRIS plans for employment, housing, health, safety, and community involvement while navigating budgets and program rules.

Successful candidates will be compassionate team players, able to work independently with strong attention to confidentiality and HIPAA compliance, and capable of coordinating with Fiscal Employer

Qualifications

• At least 2 years experience in health care, preferably in care coordination.
• Bachelor’s degree in a social work, psychology, human services, counseling, nursing, or related field.
• Valid driver's license and reliable transportation for travel requirements.

Responsibilities

• Meet in person with IRIS participants a minimum of four times per year.
• Assist participants in identifying personal outcomes within IRIS budget and DHS requirements.
• Document orientation and planning activities in the IRIS data system within 48 business hours.
