Qualifications

• Requires 2+ years in health care, care coordination, or related field.
• Bachelor’s degree in social work, psychology, human services, counseling, nursing, special ed, or equivalent.
• Valid driver’s license and reliable transportation for travel as required.

Responsibilities

• Build trusting relationships with IRIS participants and communities.
• Develop customized IRIS plans to achieve participants' goals (employment, housing, health, safety).
• Document orientation, planning, and progress in the IRIS data system (WISITs) within 48 business hours of visits.
• Educate participants on budgets and monthly reports; coordinate with Fiscal Employer Agency and IRIS Consultant Agency.
• Collaborate with staff to implement participant plans and ensure HIPAA compliance.
