Job summary

Molina Healthcare is seeking an IRIS Consultant to support Wisconsin IRIS participants remotely, with periodic in-person visits in the community. The role focuses on building trusted relationships, guiding participants to maximize their IRIS benefits, and coordinating with local resources.

Ideal candidates bring a background in social or human services, strong advocacy skills, and experience with budgets and community services.

Qualifications

At least 2 years experience in health care, preferably in care coordination, and at least 1 year serving IRIS program target groups.
Bachelor’s degree in a social work, psychology, human services, nursing, or closely related field, or four years of commensurate experience.
Valid and unrestricted driver’s license and reliable transportation for job travel.
Ability to develop and maintain professional relationships with clients, providers, and regulators.
Self-motivation and ability to work independently with minimal supervision.

Responsibilities

Meet IRIS participants in person at least four times per year, including one annual home visit.
Orient new participants to their rights, responsibilities, and program procedures.
Help participants identify personal outcomes and create customized IRIS plans.
Document activities and progress in the IRIS data system within required timelines.
Educate participants on budget reports and assist with provider billing and supports.
