Molina Healthcare is seeking an IRIS Consultant to support Wisconsin participants remotely, with in-person home visits as needed. You will help participants maximize their IRIS benefits, navigate budgets, and connect with community resources.

Qualifications

• Minimum 2 years in health care/ care coordination.
• 1 year serving IRIS program target groups or equivalent experience.
• Valid, unrestricted driver's license and reliable transportation for travel.
• Ability to work independently with minimal supervision.
• Knowledge of long-term care programs and community resources.

Responsibilities

• Meet IRIS participants in person at least four times per year; include one annual home visit.
• Provide program orientation and explain rights, responsibilities, and budgeting.
• Develop individualized IRIS plans to meet participant goals across employment, housing, health, safety, and transportation.
• Document activities in the IRIS data system within 48 business hours of visits.
• Research community resources and assist with provider billing and budget management.
