Job summary

TMG IRIS Consultant Agency is seeking an IRIS Consultant (IC) to support Wisconsin participants in navigating the IRIS program. This remote position includes home-based office work and scheduled visits in participants’ communities across Wisconsin.

The IC will build trusted relationships, help define personal outcomes, and coordinate with care teams while ensuring program compliance and budget adherence.

Qualifications

• Minimum 2 years experience in health care, preferably care coordination, and 1 year with IRIS target groups or equivalent.
• Bachelor's degree as listed or 4 years of commensurate experience.
• Valid driver’s license and reliable transportation for job-related travel.

Responsibilities

• Meet IRIS participants in person at least four times per year, with one annual home visit.
• Educate participants on IRIS program rights, responsibilities, and budgeting.
• Develop individualized IRIS plans to achieve participant goals.
• Coordinate with Fiscal Employer Agency and IRIS providers.
• Document activities in the IRIS data system within 48 business hours of visits.
• Research community resources to support participant goals and share information.
• Maintain confidentiality and HIPAA compliance.
• Attend monthly team meetings and weekly check-ins as required.
