Job summary

Molina Healthcare is seeking an IRIS Consultant (IC) to partner with participants enrolled in the Wisconsin IRIS program. This remote, home-based role requires regular in-person visits with participants in their homes and community, as you help them navigate long-term care options and achieve personal goals.

Qualifications

• At least 2 years experience in health care, preferably in care coordination, and at least 1 year of experience serving IRIS program target groups.
• Bachelor’s degree in a social work, psychology, human services, counseling, nursing, special education, or a closely related field (or four years of commensurate experience if no degree).
• Valid driver’s license, reliable transportation, and adequate auto insurance for job-related travel requirements.
• Ability to develop positive work relationships with coworkers, clients, and regulators.
• Ability to work independently with minimal supervision and self-motivation.
• Knowledge of long-term care programs and self-determination principles.
• Problem-solving and critical-thinking skills.
• Excellent time-management and prioritization skills.
• Ability to handle multiple projects and adapt to change.
• Ability to maintain professional relationships in diverse settings.
• Knowledge of community resources.
• Proactive and detail-oriented.
• Excellent verbal and written communication skills.

Responsibilities

• Meet in person with IRIS participant at least four times per year and attend annual home visit.
• Provide program orientation to new participants and explain rights and responsibilities.
• Identify personal outcomes and align with IRIS budget and plan.
• Document orientation and planning activities within the IRIS data system within 48 business hours.
• Research community resources and natural supports to fit participant outcomes.
• Maintain confidentiality and HIPAA compliance in all activities.
• Collaborate with IRIS staff to implement participant plans and coordinate with agencies.
• Attend in-person monthly team meetings and participate in regular check-ins.
