Job summary

TMG IRIS Consultant Agency seeks an IRIS Consultant to support participants in Wisconsin’s IRIS program through remote coordination and home-visit engagement. You will build strong relationships, assess goals, and create customized plans within IRIS budgets.

Responsibilities include participant orientation, budget literacy, and resources navigation, with regular team meetings and direct support from supervisors. This role requires in-person visits and community-based work.

Qualifications

• Minimum 2 years of health care experience, preferably in care coordination.
• Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or related field (or 4 years of commensurate experience).
• Valid driver’s license and reliable transportation for travel requirements.

Responsibilities

• Build engaged relationships with IRIS participants and help them navigate the IRIS program.
• Identify personal outcomes and develop customized IRIS plans within the client’s budget and DHS guidelines.
• Document orientation and planning activities in the WISITs data system within 48 business hours.
• Research community resources and natural supports to match participant outcomes.
• Ensure program changes and compliance are communicated effectively to participants and care teams.
• Attend monthly in-person meetings and weekly check-ins with IC supervisor.
