Job summary

Modivcare Personal Care Services is seeking an Intake Coordinator for private-pay home care, primarily remote from home. Opportunity to support clients from initial inquiry through successful start of care, with pay of $24.68-$26.50/hour depending on experience.

Responsibilities include handling inquiries, conducting intake conversations, educating clients on services and onboarding, coordinating assessments and caregiver matching, and maintaining accurate intake records throughout the process.

Qualifications

• 1+ years of direct home care intake or care coordination experience.
• Private-pay home care experience required.
• Scheduling and staffing including caregiver matching and start-of-care coordination.
• Experience providing high-touch service to clients and families.
• Bilingual English/Spanish strongly preferred.
• Strong communication, organization, and computer skills.

Responsibilities

• Manage new private-pay inquiries and referrals with prompt, high-touch support.
• Conduct intake conversations to understand care needs, schedules, location, and start dates.
• Educate clients on services, rates, and the private-pay onboarding process.
• Maintain follow-up with prospective clients to move inquiries toward onboarding.
• Coordinate assessments, documentation, caregiver matching, staffing, and start-of-care.
• Maintain intake records from inquiry to start of care.
