Job summary

CenterWell is seeking an Intake Coordinator to handle calls from referrals and patients, screen for home care eligibility, and coordinate transitions with the clinical team.

You will verify benefits, negotiate rates, and obtain initial authorizations while documenting conversations and maintaining payer relationships. This remote role may require occasional travel to Humana offices for training.

Qualifications

• High School Diploma or equivalent.
• Typing speed 40 wpm or higher.
• Proficiency with Microsoft Office applications including Word, Excel, Outlook, and Teams.
• 3+ years in health care delivery or related business experience preferred.
• Knowledge of medical terminology preferred.
• Knowledge of insurance reimbursement process preferred.
• Data entry experience preferred.

Responsibilities

• Receive and respond to calls from referrals or patients to identify needs and determine ability to meet them.
• Record outcomes and follow up, identify alternative community resources when needed.
• Perform initial telephone screening for home care eligibility and inform the clinical team of acceptance.
• Notify referral sources of status and provide information to ensure smooth patient transition to home care.
• Identify payer sources, verify benefits, negotiate rates, obtain authorizations and establish primary payers.
• Access account information and communicate terms to staff.
• Collaborate with Intake team to resolve payor issues and monitor referral source satisfaction.
