Job summary

Opco-Skilled-Management- is seeking a Central Intake Nurse to coordinate initial and ongoing referral documentation from physicians, insurers, and providers to determine service appropriateness and facility placement. You will develop patient care plans with defined frequency and duration and initiate care with service providers.

The role requires a current state nursing license, at least two years in healthcare administration at a managerial level, and strong coordination skills to streamline

Qualifications

• High school diploma required.
• Current nursing license for the appropriate state (NM).
• Two years of healthcare administration at the managerial level required.

Responsibilities

• Consolidates and coordinates inbound referrals for admissions and care planning.
• Verifies insurance coverage and aligns patient needs with available facilities.
• Manages and improves the admissions process and data integrity in MIS systems.
• Acts as the primary contact to guide patients and families through services and community supports.
• Collects data and signs admission documents to ensure a smooth transition of care.
• Reviews data for reports and assists in maintaining CRM/IRM/PCC data accuracy.
• May assume additional duties or temporary vacancies as needed.
• Other duties may be assigned with or without notice.
