Job summary

UnitedHealth Group is seeking a Central Intake Coordinator in Home Health to provide administrative support by triaging calls and entering referrals. The role involves coordinating with Central Intake Teams, branches, and nurses to ensure accurate patient data and smooth referral workflows.

The position is office-based with full-time hours and a comprehensive benefits package. Candidates should have an LPN license and at least one year of related office and insurance/medical experience.

Qualifications

• LPN licensure in the state of practice.
• 1+ years experience in office procedures.
• 1+ years Insurance/Medical experience.

Responsibilities

• Answers phones and triages calls to Central Intake Teams.
• Communicates patient/referral information to Central Intake Staff and branches.
• Obtains demographic and clinical information to compile referrals.
• Coordinates communication of referral information with Central Intake Nurses.
• Transfers high tech/nurse specific referrals to Central Intake Nurses.
• Triages incoming calls and messages and relays as appropriate.
• Disseminates patient ping data to PCM, field clinicians, or sales reps.
• Attends workshops and training programs to stay updated.
