Job summary

UnitedHealth Group is seeking a Central Intake Coordinator in Home Health to provide administrative support by triaging calls and entering referrals into the operating system. You will coordinate with Central Intake Teams, branches, and nurses to ensure accurate, timely referral information and patient data entry.

The role requires LPN licensure and at least 1 year of experience in office procedures and insurance/medical settings.

Qualifications

• LPN licensure in the state of practice.
• 1+ years experience in office procedures.
• 1+ years Insurance/Medical experience.

Responsibilities

• Answers phones and triages calls to Central Intake Teams.
• Effectively communicates patient/referral information to Central Intake Staff and branches.
• Obtains demographic and clinical information from clients and sources to compile referrals.
• Coordinates communication of referral information with Central Intake Nurses.
• Transfers High Tech/nurse specific referral calls to Central Intake Nurses.
• Triages incoming calls and messages from referral sources and relays messages as appropriate.
• Disseminate patient ping data to PCM, field clinicians, or sales representatives as applicable.
• All other duties as assigned.
